Every first-time patient is required to fill out a set of new patient forms. You can access these forms by mail, online through our patient portal, or in person by arriving at your appointment about 15 minutes early.

For our patients’ convenience, we currently accept most major insurance plans (including Medicare) and cash payments for those without insurance.

You can schedule your virtual appointment through our convenient mobile app, Spruce. If you need to cancel an existing appointment, please contact us within 24 hours of your appointment time.

Yes, we do offer Occupational Therapy services. Our Occupational Therapist, Rebecca Burkett, graduated from UTC in with an Occupational Therapy Doctorate (OTD) and has experience in acute care. She also has a background in psychology/mental health, completing a OTD capstone project working with adults with dual diagnoses of mental illness + physical/developmental disabilities.
